In-Office Insurance Plans & Specials

DISCLAIMER: This is not an insurance plan and cannot be combined with other insurance or discount plans. Both dentist and patient are prohibited from billing insurers/filing insurance claims for services covered by the in-office plan. The annual plan fee must be paid in full on the date of sign up. All appointments for services covered by the plan, excluding 1 emergency exam, must be scheduled on the date of sign up. Appointment for 1 emergency exam must be completed within 12 months from the sign up date. No refunds will be given in the event the patient terminates the plan prior to the end of the plan year. Covered services not used within the specified time period will not be carried over or transferred. This plan is for individual use only and is not a group benefits plan. Plan benefits can only be used by 1 person, are not transferable, have no cash value and may not be redeemed for cash. Services will be provided at sign-up location only. Patients are responsible for notifying dental practice of any address or contact changes. Missed appointment fees apply. Brighter Dental retains the right to interpret any program stipulations.
